Do we need to be an established patient?
No. The clinic offers sports physicals without a new-patient intake visit.
What should we bring?
Bring the completed AIA Pre-Participation Physical Evaluation and Medical Eligibility forms, plus any relevant health records or medication list.
How much is the exam?
The published price is $45 for non-established patients, $20 for established patients, and included for members.
