INVESTING IN YOU

At Max-imum Well-ness Integrative Health Clinic, we choose to put all of our time and resources into providing you with the best care possible. In order to do that, we do not bill insurance on your behalf. All visits have a defined cash pay price. Payment is due at the time of scheduling for all New Patients and at the time of service for established patients. Although we do not bill insurance for you, we will happily provide you with all necessary insurance documents for you to submit to your provider. Reimbursement for your visit is dependent on your insurance plan.

New and Established Patient Policy

  • New Patient Visit: A patient is considered “new” if they have never been seen at our clinic before or if it has been more than 12 months since their last visit. All new patients are required to complete initial intake paperwork, a comprehensive consultation, and pay the one-time enrollment fee at the time of their first visit.
     
  • Established Patient Visit: A patient is considered “established” if they have been seen within the past 12 months and are returning for follow-up care, treatment, or wellness services. Established patients do not pay the enrollment fee again as long as care is continuous.
     
  • Re-Establishing Care: If more than one year (12 months) has passed since a patient’s last visit, they will be considered a new patient again. In this case, they will need to complete updated intake paperwork, schedule a new patient consultation, and pay the enrollment fee again before services can resume.

CANCELLATION POLICIES

New Appointment Cancellation Policy


- 4 Business Days before your scheduled appointment you will be 

reimbursed 100%.

- 3 Business Days or less before your scheduled appointment you will be reimbursed 50% of your booking fee.

- If you cancel the day of your appointment or are a No Show you WILL NOT be reimbursed. 


Follow Up Cancellation Policy


- If you cancel 2 Business Days before your scheduled appointment there will be no charge.

- If you cancel 2 Business Days or less before your scheduled appointment, you will be charged 50% of the appointment fee.  

- If you cancel the day of your appointment or are a No Show you will be charged the full appointment fee.
