• New Patient Visit: A patient is considered “new” if they have never been seen at our clinic before or if it has been more than 12 months since their last visit. All new patients are required to complete initial intake paperwork, a comprehensive consultation, and pay the initial fee at the time of their first visit.
   
• Established Patient Visit: A patient is considered “established” if they have been seen within the past 12 months and are returning for follow-up care, treatment, or wellness services. Established patients do not pay the initial fee and will pay the follow up fee as long as care is continuous.
   
• Re-Establishing Care: If more than one year (12 months) has passed since a patient’s last visit, they will be considered a new patient again. In this case, they will need to complete updated intake paperwork, schedule a new patient consultation, and pay the initial fee again before services can resume.

All Maximum Wellness Integrative Health memberships are billed automatically each month until canceled. Monthly Wellness Wallet credits are added after successful payment and may be applied toward eligible medical services, treatments, supplements, and other approved purchases.

Membership Plans:

• Single — $150/month: $90 monthly wallet credit.
• Couple — $200/month: $120 monthly shared wallet credit.
• Family — $300/month: $180 monthly shared wallet credit.
• Hormone — $99/month: $99 monthly wallet credit.

Unused wallet credits roll over each month. Couple and Family memberships share one wallet among authorized enrolled members.

Single, Couple, and Family memberships include one eligible follow-up visit per month per membership account. Additional visits may be paid using available wallet credits. Hormone Membership benefits and visit coverage are determined by the applicable hormone care plan.

Members receive 25% off eligible supplements, peptides, IV therapies, and procedures. Discounts and wallet redemption are subject to clinical eligibility and applicable restrictions. Laboratory testing, medications, and services not expressly included may incur additional charges.

Cancellation: Members may cancel according to their signed Membership Agreement. Future billing stops when cancellation becomes effective. Previously accumulated wallet credits will be handled according to the agreement and applicable law.

Membership is not health insurance, does not guarantee specific treatments, and requires a signed Membership Agreement. All medical care is subject to physician evaluation and clinical appropriateness.

 Appointment Cancellation Policy

We kindly ask that all patients reschedule or cancel at least 24 hours before their appointment.

Any cancellation made less than 24 hours before the scheduled appointment time maybe subject to a $50 fee.

This includes same-day cancellations and no-shows.
